Medicare and
Medicare/Medi-Cal
If you are insured by Medicare, CCPM&R
is a participating Medicare provider, and as such, does accept Medicare
assignment. Medicare will be billed directly for all of the services
rendered. Although we accept Medicare assignment, you are responsible for
the federal co-payment as directed on you explanation of benefits from Medicare
unless you are eligible for Medi-Cal benefits.
Medicare and Secondary
Insurance
If
you have coverage with a SECONDARY
INSURANCE, it is your responsibility
to notify the business office and provide the insurance name, address,
subscriber's name, subscriber's certificate, and group number. Medicare
will be billed as well as your secondary insurance. When all payments have
been received from your insurance carriers, you will receive a letter indicating
a payment is required by you and the reason it is required.
Private Insurance
As a courtesy to you, the business office will bill your
private insurance. Patients with PPO coverage may have co-payment
requirements and are expected to remit payment at time of service. All
questions regarding late payments and policy coverage should be discussed by you
with your insurance representative. We do not wait for insurance to pay. Payment
is due 30 days from the receipt of your first statement.
If there is a delay or dispute with your insurance carrier, suitable monthly
payments can be arranged with the business office. You
are responsible for payment in full, regardless of your insurance coverage.
Work Related Injury
You
are responsible for furnishing complete worker's compensation information to our
billing office. Upon receipt of the necessary information, the industrial
carrier will be billed directly for the physician services rendered.
